  How can I obtain my birth certificate?

If you were born between 1910 and the present, a certified birth certificate is available through the Department of Health and Mental Hygiene via the internet (www.nyc.gov/health); for credit cards: telephone (212) 788-4505, fax (800) 908-9146; mail, or in-person from 125 Worth Street, Room 133 Brooklyn, NY 10013. The cost is $15. per certified copy (other fees such as mailing and surcharges may apply). Birth Certificate Application forms and instructions may be also be obtained from the Borough President’s Community Service Center by calling (718) 802-3777. For records prior to 1910, please contact the Municipal Archives at 31 Chambers Street, Room 103, New York, NY 10007. You may also access the Municipal Archives' Website at www.NYC.gov.

I reside in Brooklyn. How can I obtain a Social Security Card?
The Social Security Card Center is located at 10 MetroTech Center (625 Fulton Street) in downtown Brooklyn.
The center is the only location where residents of Brooklyn, both U.S. citizens and non-citizens, can apply for an original Social Security Number or for a replacement card.
The Brooklyn Social Security Card Center will process only Social Security card applications. For all other Social Security services, such as retirement, disability or survivor's benefits and Supplemental Security Income, Brooklyn residents should visit their local Social Security office, call 1-800-772-1213 or visit the Social Security Administration website at http://www.socialsecurity.gov
